Registered Serviceperson and Agency is a mandatory program in Ohio to perform any type service to a commercial device.

Registered Serviceperson Application

The Registered Serviceperson Application is used for new, renewal, or replacement information within Ohio's Registration of Servicepersons.

These documents must accompany your application: 

  • A calibration certificate for equipment in each category you wish to be registered.
  • The form used by your company to place a device into service for each category.
  • The Test date and time you would like to take the exam.

There is a $25 application fee and a $75 fee after the exam is passed with an 80 percent or above. 

2020 Registered Serviceperson Exam

Testing is from 9:00 to 12:00 p.m. and 1:00 to 4:00 p.m. on the scheduled dates below, in the Weights & Measures Building at 8995 E. Main Street, Reynoldsburg, OH. All necessary forms and calibration certificates must be submitted before the testing date and time requested will be confirmed.

Participants must score an 80 percent or higher to pass the exam. 

"No shows" will be required to reapply and submit another $25 application fee.

No cell phones are permitted in the classroom.

You will be allowed to use a laptop only if the study material has already been downloaded. You will not be permitted to go online and download the material during the test. Handbooks with the study material will be available in the classroom for your use. You will be asked to wear gloves while using these handbooks.

Exam Registration

Registration should be scheduled as far in advance as possible. Class size is currently reduced from 16 to six participants, so registering early is recommended. 

Fill in the registration form, choose an exam date and time, and return the completed registration and all necessary forms to:

All December testing dates are full, however, feel free to call the office to see if there have been any cancellations.

When finished with the test, you will exit the room and premises. All tests will be graded at a later time and you or your office will be notified of your score. Payment arrangements can be made at that time. No checks or credit cards will be needed on the day of the test.

Retakes

Participants scoring less than 80 percent will need to resubmit another application and $25 payment to retake the exam.

Ohio Broken Seal Form

Ohio Broken Seal Form is to be used on all commercial meters that a seal has been broken. 

This includes the following devices:

Retail Motor Fuel Dispensers
Kerosene Dispensers
High Flow Diesel Meters
Vehicle Tank Meters
LPG Meters
Bulk Rack Meters
CNG Meters
Agri-Chemical Meters
Grain Moisture Meters

Registered Service Person Agency Application

The Registered Service Agency Application should be used to apply for a New, Renewal or Replacement Service Agency.

This application should include the following:

  1. A calibration certificate for equipment in each category you wish to be registered.
  2. The form used by your company to place a device into service in each category you wish to be registered in
  3. You must have at least one registered serviceperson to apply for the registration.
