The Registered Service Agency Application should be used to apply for a New, Renewal or Replacement Service Agency.

This application should include the following:

1. A calibration certificate for equipment in each category you wish to be registered.
2. The form used by your company to place a device into service in each category you wish to be registered in
3. You must have at least one registered serviceperson to apply for the registration.
