SWCD Board Member Conflict of Interest Information Form

Newly-elected or newly-appointed SWCD board members should complete this form, if after reviewing the Ohio ethics law, they believe they may have a potential conflict of interest with regard to their service on the SWCD board of supervisors. Use the  Conflict of Interest Information Form to identify and describe any potential or real, family or business relationships that may be conflicts of interest under Ohio ethics law. Review the document with your ODA Division of Soil and Water Conservation designated Program Specialist for guidance on further action to resolve or mitigate the conflict.
